Ausgangssituation
Das deutsche Bundesministerium für wirtschaftliche Zusammenarbeit und Entwicklung (BMZ) unterstützt seit 1994 die Entwicklung des nepalesischen Gesundheitssektors. Die in den letzten Jahren begonnenen Vorhaben zur sozialen Absicherung im Krankheitsfall in Nepal sind nach wie vor unterentwickelt, sodass die Bürger nur unzureichend vor katastrophalen Gesundheitsausgaben geschützt werden und die Ungleichheiten beim Zugang zu einer guten Gesundheitsversorgung fortbestehen. Des Weiteren verhindern systemische Defizite die Modernisierung – insbesondere im Bereich der Digitalisierung, die entscheidend für die qualitative Verbesserung der Gesundheitsversorgung und -vorsorge ist.

Ziel
Die institutionellen Kapazitäten staatlicher Akteure im Gesundheitssystem – insbesondere des Ministeriums für Gesundheit und Bevölkerung, der Krankenversicherungsbehörde und ausgewählter kommunaler Gesundheitsbehörden in zwei Provinzen – wurden mit dem Ziel ver-bessert, Nepals Strategie für den Gesundheitssektor (Nepal Health Sector Strategic Plan) umzusetzen.

Vorgehensweise
S2HSS II zielt darauf ab, die Fähigkeiten der wichtigsten Akteure im Gesundheitssektor zu stärken – insbesondere des Ministeriums für Gesundheit und Bevölkerung (MoHP), des Health Insurance Board (HIB) und beispielhaft ausgewählter subnationaler Gesundheitsbehörden in zwei Provinzen. Dies soll eine gerechte und hochwertige Gesundheitsversorgung für alle gewährleisten. Aufbauend auf vorigen Erfolgen konzentriert sich das Programm auf drei Hauptbereiche:
a) Stärkung der Verwaltungskapazitäten und Verbesserung der finanziellen Nachhaltigkeit des Nationalen Krankenversicherungsprogramms.
b) Förderung eines evidenzbasierten Gesundheitsmanagements durch datengestützte Entscheidungsfindung und Bürgerbeteiligung. Dies soll zu einer effizienten sektoralen Planung auf subnationaler Ebene führen.
c) Die Stärkung digitaler Gesundheitsinformationssysteme.

Context
The German Federal Ministry for Economic Cooperation and Development (BMZ) has sup-ported Nepal’s health sector development since 1994. Nepal’s social health security schemes, introduced in recent years, remain underdeveloped, leaving citizens inadequately protected and perpetuating inequities in healthcare access. Moreover, systemic deficits hinder modernization, particularly in digitalization, crucial for im-proving health service delivery.

Objective
The institutional capacities of state actors in the health system, in particular the Ministry of Health and Population, the Health Insurance Board and selected municipal health authorities in two provinces, to implement the Nepal Health Sector Strategic Plan have been enhanced.

Approach
S2HSS II aims to strengthen the abilities of key actors in the health sector, particularly the Ministry of Health and Population (MoHP), the Health Insurance Board (HIB), and selected sub-national health authorities in two provinces, to provide equitable and quality healthcare services for everyone. Building on past achievements, it focuses on three main areas:
a) Strengthening organizational management and improving the financial sustainability of na-tional health insurance program, that involves supporting HIB to develop strategies for sus-tainable financing and efficient claims management, as a foundation to increase insurance coverage among poor and vulnerable populations.
b) Promoting evidence-based health management through data-driven decision-making and citizen participation, leading to efficient sectoral planning at sub national level and
c) Strengthening digital health information systems by establishing standards, ensuring in-teroperability and allowing efficient data exchange leading to better data-driven decision-making, reduced costs, and enhanced public health outcomes.
